When you start care at our office, you will need to fill out some paperwork so we may truly know your health status. You can download and fill out the paperwork before coming for your first visit. Please download the appropriate history form and the accompanying Privacy form.
We are located West of the post office, at 53 King St West.
Hours
- Monday and Wednesday: 9:00 - 12:00, 3:00 - 6:30 pm
- Tuesday and Thursday: 4:00 pm - 8:00 pm
- Friday: 9:00 - 12:00 noon
